While performing an upgrade to Plesk 10.3 with either the GUI updater or the command line, /usr/local/psa/bin/autoinstaller, users receive the following error message:
Trying to set up APS controller database... Attempting to check APS db connection via ODBC, using /usr/bin/isql (empty output is good)
[ISQL]ERROR: Could not SQLConnect
Failed, rc = 1.
Failed to establish test connection. Cleaning up.

ERROR while trying to establish test connection to APS database via ODBC
Check the error reason(see log file: /tmp/plesk_10.3.0_installation.log), fix and try again

This issue is caused by mysql not being able to accept network connections:

[root@domain ~]# mysql -h -P 3306 -uadmin -p`cat /etc/psa/.psa.shadow`
ERROR 2003 (HY000): Can't connect to MySQL server on '' (111)

ODBC connects to mysql via the local TCP/IP connection on port 3306.

Comment out a line in /etc/my.cnf to enable it

After that, repair will run with no issue.
