Server crashes with BSoD IRQL_NOT_LESS_OR_EQUAL (a).
The dump check shows the following results:

Probably caused by : PCIIDEX.SYS ( PCIIDEX!BmSetup+6b )

The problem is caused by the MS Windows issue described in this article: KB#943170.
NOTE: The fix provided in KB#943170 is not applicable to Windows Server 2008.


It is suggested to contact Microsoft and your hardware vendor for the SATA controller in order to fix the issue.

The following workaround is also known to work:
  1. Make sure all Windows programs and Parallels Virtuzzo Container containers are stopped.
  2. Locate the registry HKEY_LOCAL_MACHINE\SYSTEM\CurrentControlSet\services\msahci.
  3. Right-click Start in the Name column.
  4. Set Value data to "0" as per this Microsoft KB article: KB#922976.
  5. Apply the changes, and reboot the server.
  6. While booting, go to BIOS.
  7. Switch SATA mode to "AHCI."
  8. Apply the BIOS changes.
  9. Boot up the server.
