System tries to load module which does not exist physically on the server.

  1. Locate present in PHP configuration file:

    # egrep -ir "opcache" /etc/php/*
  2. Make sure that such library exists phisically on the server.

  3. If library is absent, disable/comment module in configuration file (/etc/php5/mods-available/opcache.ini) due to such module is not found.
