Plesk upgrade failed with:

ERROR while trying to establish test connection
Check the error reason(see log file: /var/log/plesk/install/plesk_12.5.30_installation.log), fix and try again

Ensure that MySQL config file /etc/my.cnf do not have skip-grant-tables enabled

# grep skip-grant-tables /etc/my.cnf

Verify that you are able to connect to MySQL with Plesk admin password

# mysql -uadmin -p`cat /etc/psa/.psa.shadow` -p 3306
# mysql -uadmin -p`cat /etc/psa/.psa.shadow`

Plesk not able to connect to MySQL server

If you've got an Access Denied error, then you should reset Plesk 'admin' password in MySQL.

  1. Add skip-grant-tables into '/etc/my.cnf'
  2. Get password from '/etc/psa/.psa.shadow'
  3. Reset admin password in MySQL

    ~]# mysql -uadmin mysql
    mysql> begin;
    mysql> update mysql.user set password=password('<PASSWORD FROM /etc/psa/.psa.shadow>') where user='admin';
    mysql> commit;
