Resolution

For version 3.x should be applied patch feedback_module_patch_v.3.x.tar.gz in following way:

tar -xzvf feedback_module_patch_v.3.x.tar.gz -C <sb-root>

After applying patch, new module must be created on the site.

For version 4.0 should be applied patch feedback_module_patch_v.4.0.tar.gz in following way:

tar -xzvf feedback_module_patch_v.4.0.tar.gz  -C <sb-root>

After applying the patch, full site republication must be done.

For version 4.1 should be applied patch feedback_module_patch_v.4.1.tar.gz in following way:

tar -xzvf feedback_module_patch_v.4.1.tar.gz  -C <sb-root>

After applying the patch, full site republication must be done.

 Where <sb-root> is /usr/local/sitebuilder/ or /opt/sitebuilder (on Debian servers).

As for 4.2 version patch already included into main build, so there is no need to apply any additional patches if you have 4.2 version installed.
